To provide services to the veteran population enrolled at the James H Quillen VA Medical Center, primarily Sevierville Outpatient clinic. Services include provision of high-quality case management, clinical treatment, advocacy, and coordination of linkages to other appropriate VA or community service providers / agencies as needed by the Veteran.

Duties

Please note this position will require 50% or more of local community travel. Duties as the Social Worker Program Coordinator (HBPC Satellite) include: Serves as coordinator of the the Home Base Primary Care Program. Plans and coordinates all elements of the Home Base Primary Care Program, including delegation of work to staff assigned to the program. Responsible for independently assuring that VA Central Office HBPC program standards and policies are adhered to in a timely and effective manner. Manages the daily operation of the HBPC Program. Develops policies and procedures for program operation. Organizes and prepares reports and statistics for service leadership, the health system director, VA Central Office and others. Coordinates interdisciplinary staff work assignments. Ensures that monthly follow-up visits to veterans are completed by the HBPC Social workers and HBPC Liaisons, as required in program guidelines. Assists with establishing and maintaining the annual HBPC budget. Collaborates with Business Office staff to ensure that payments to HBPC facilities are made in a timely manner and in compliance with VA standards. Serves as liaison to community agencies and networks with agency representatives. Independently conducts psychosocial assessments of a wide variety of individuals from various socioeconomic, cultural ethnic, educational and other diverse backgrounds. Other duties as assigned. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Grade Determinations: Social Worker Program Coordinator GS 12 (1) Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. References: VA Handbook 5005/120 PART II APPENDIX G39 The full performance level of this vacancy is GS-12.
