One permanent position is open at the Western Oregon Service Unit, an ambulatory health care center, Salem, Oregon. Work is in the INTEGRATED SERVICES section of the Behavorial Health Program and reports to the Behavorial Health Director. Position functions as a Behavioral Care Manager who is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient's medical provider and care team.

Duties

Assess and evaluate patients using a student/patient/family-centered approach for needs such as financial assistance, discharge planning/coordination, long-term care placement, and other psycho-social concerns related to health care. Develops intervention based on above need assessment and exercise mature professional judgement in consultation with the supervisory social worker in providing problem-focused, culturally appropriate care. Participate and when appropriate takes a leadership role in assessing community human service needs. Works with community members and human resources providers to develop approaches to addressing the needs. Assist in monitoring and reviewing services for quality and appropriateness of care. Assists in developing as needed appropriate services to address patient and family psycho-social needs. Prepares reports as requested on quality improvement activities.

Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT(S): Psychology Series, 0180 Degree: Clinical psychology: For positions at grades GS-11 and above, satisfactory completion of all the requirements for the doctoral degree (Ph.D. or equivalent) directly related to full professional work in clinical psychology is required. Social Work Series, 0185 Degree: Social Work: Must fulfill all of the requirements for the master's degree in social work in a school accredited by the Council on Social Work Education. Master's degree with practicum: Applicants who complete all of the requirements for a master's degree in social work that included a practicum are eligible for GS-9. In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS FOR THE GS-11 grade: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: work experience in preventing, treating, alleviating and/or mitigating behavioral health problems which manifest themselves as suicides, attempted suicides, drug abuse, addiction, alcoholism, mental health disorders, physical and mental injury to self and others, and chronic healthcare disorders and the correlation between behavioral health disorders based on related professional knowledge of current theories and practices such as integrated behavioral health practices. SELECTIVE PLACEMENT FACTOR REQUIREMENT In addition to the minimum requirements, the following selective placement factor is determined to be essential to perform the duties and responsibilities of the position. Licensure Requirement, GS-0185, Social Worker This position requires a current, unrestricted U.S. State license as a Licensed Independent Social Worker (LISW), a Licensed Clinical Social Worker (LCSW), or a Certified Independent Social Worker (CISW). Licensure Requirement, GS-0180, Clinical Psychologist This position requires a current, unrestricted U.S. State license as Licensed Clinical Psychologist. Please provide a copy of your license with your application. Applicants who do not meet a selective placement factor are ineligible for further consideration.
