As M2VA Program Coordinator you oversee all transition activities and coordination of care and services for service members and Veterans treated at the facility who were discharged from the military after 9/11/2001. Their health care and psychosocial needs are complex and require a high degree of clinical oversight and problem solving. You will facilitate delivery of services across episodes of care within the Medical Center & Community-based Outpatient Clinics, including Mental Health Services.

Duties

The M2VA Program Coordinator may supervise M2VA nurse and social worker Case Managers, PACT/HBPC Social Workers and facility TPAs. Oversight includes, but is not limited to, the following duties: (1) Ensures that all Post 9/11 service members and Veterans are screened for the need for care management services and that those in need of nurse and/or social worker Case Managers are assigned accordingly, particularly those who are severely-injured or ill. FS-SWS Supervisor GS-0185-12 (2) Coordinates psychosocial evaluation, assessment, and periodic reassessments of Post 9/11 patients served, which provides a comprehensive database to identify psychological, social, and vocational needs and the appropriate treatment and services to be provided. (3) Coordinates provision of individual and family treatment and support services provided to Post 9/11 service members and Veterans. (4) May oversee the TPA, including monitoring their caseload and the advocacy services provided to Post 9/11 service members and/or Veterans in their caseload. (5) Advocates on behalf of the Post 9/11 service member and/or Veteran to ensure that services and benefits are obtained in a timely manner and in keeping with the VA's goal for excellence in customer service. (6) Manages, coordinates, and is accountable for the provision and overall effectiveness of care management, family support services, and referrals for prosthetic appliances and home equipment for Post 9/11 service members and/or Veterans. (7) Works collaboratively with staff at Vet Centers and the Veterans Benefits Administration (VBA) Regional Office. (8) Maintains a current network of referral resources to include substance abuse treatment, outpatient medical and/or psychiatric care, vocational rehabilitation, etc. and with VSOs. (9) Works with PACT team leadership to ensure Veterans assigned in all PACTS receive appropriate case management services. C. Patient, Stakeholders, and Staff Education (1) Provides consultation to other staff on the special needs of Post 9/11 service members and Veterans. (2) Provides education to Post 9/11 service members and Veterans on VHA health care services available at the facility and about services available within the VISN and in the local community. (3) Establishes and maintains an ongoing education program for patients, community agencies, students, and staff to facilitate understanding of medical treatment, including clinical practice guidelines, mental health issues, readjustment issues, psychosocial problems, and care giver issues facing the Post 9/11 population. D. Other duties as assigned. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Please note: A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency. Social workers must be proficient in spoken and written English in accordance with VA Regulations. Must pass a background check. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: (1) Education and Experience. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. [(3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Ability to independently organize work, set priorities, and meet multiple deadlines. (b) Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. (c) Ability to ensure provision of clinical social work services. (d) Ability to provide staff training and development. (e) Skill in interpersonal relationships in dealing with employees, team leaders, and managers. (4) Assignment. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. At the GS-12 grade level, supervisory social workers typically have significant personnel management responsibilities for a small to moderate sized professional and non-professional staff, including support staff and professional staff at the GS-12 grade level and below. The supervisory social worker is responsible for the professional and administrative management of an area in a social work service, in a care/product line, or multiple facilities. They provide direct clinical services with or without program coordination responsibilities.] Preferred Experience: Must have LICSW and have ability to provide supervision to social workers trying to attain a license.
