The HUD/VASH Social Worker/Program Coordinator manages and is the administrator of the HUD/VASH Program which re-integrates homeless Veterans into the community to help them achieve the maximum level of physical, mental, and social functioning possible and is located at the West Haven VA Medical Center in West Haven CT.

Duties

Duties include but are not limited to: Responsible for submission of statistical data to Homeless Operations Management and Evaluation System (HOMES) for computation Oversight and tracking of medical, mental health, and substance use care access and follow up, as well as Veteran satisfaction and service outcomes. Provide leadership within and outside of the VA to develop programs with VA and community partners that will better serve the homeless veteran population. Reviews and implements clinical practices that are in accordance with the current best practices with guidance from the Commission on Accreditation for Rehabilitation Facilities (CARF) accreditation process and the Joint Commission's policies and reviews. Responsible for administrative and clinical supervision of the HUD/VASH Social Work Supervisors and ultimately their direct reports of HUD/VASH Social Workers, peer support apprentices/specialists and assigned administrative staff. Responsible for conducting interviews and the selection of new staff members in cooperation with HUD/VASH team member, Homeless Director and supervisory staff as indicated. Responsible for conducting team meetings within the various program areas and for the overall HUD/VASH program. Responsible for maintaining and/or developing action plans that will maintain and/or increase the compliance of monitors and reporting on the outcomes to VA Connecticut leadership, including Homeless Director, ECCC and Mental Health Leadership as well as the VISN. Responsible to collaborate with other services, Quality Assurance, Mental Health, Errera Center, Homeless and VA leadership through review of accreditation standards and policy implementation of these standards within the program. Works with a variety of agencies and staff at multiple sites, including the VA at Newington and West Haven to further collaboration and education related to VA resources, policies, procedures, and continual building of partnering opportunities. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a Master of Social Work. Licensed or certified by a state to independently practice social work at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. Social Worker- Program Coordinator GS-13 Experience: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in coordinating and implementing one or more specialty program(s), across the facility. Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Knowledge of organizational structure and impact on program operations. Preferred Experience: Community experience preferred Experience with homeless population, serious mental illness and/or substance use. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-39 Social Worker Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Traveling throughout the Medical Center and the community is required, as is performing activities involving sitting, walking, and standing, bending, and carrying such items as books, papers, and files. In carrying out responsibilities it may be necessary for the incumbent to travel into the community where he/she conducts interviews with the Veterans, their families, representatives of community health and welfare agencies and law enforcement agencies.
